Probing cognitive control neurocircuitry: a concurrent TMS-fMRI investigation

Objective:In the current study, we investigated whether probing three superficial cortical nodes of the cognitive control network, would differentially influence the responsiveness of the fronto-parietal and cingular-opercular subnetworks that comprise the broader network.Methods:Twenty-four healthy individuals completed a concurrent TMS-fMRI session, with spTMS delivered to left and right dorsolateral prefrontal cortex (dlPFC) as well as pre-supplementary motor area (preSMA). Order of stimulation site was counterbalanced across participants.Results:Single pulse TMS delivered at 120% of motor threshold to the left and right dlPFC as well as pre-SMA resulted in significant activations of both the fronto-parietal and cingular-opercular subnetworks, as well as striatal and somatosensory regions. While pre-SMA stimulation resulted in greater activation of the somatomotor network, effects secondary to spTMS to the three nodes were largely indistinguishable.Conclusions:Therapeutic rTMS is most commonly applied to the left dlPFC toward the aim of improving cognitive and emotional regulation. The current findings suggest that other superficial cortical regions of the cognitive control network, specifically contralateral right dlPFC and pre-supplementary motor area are similarly robust targets for influencing the interconnected network. These findings are especially important for neurodegenerative conditions such as stroke for which targeting must be optimized for cortical anomalies and associated network disruptions.
